Can anyone provide reference of an excellent dentist off camp? I need to regular teeth checkup and cleaning.

We always go to the Rabwa Dental Clinic. It's behing the Holiday Inn off the Khobar Dammam Highway (if you're coming from Doha, it's left after the first flyover). They're open 6 days a week till 9PM so getting an appointment is not a nightmare like the one in camp.
Their prices are reasonable though I have noticed that it has gone up in prices (probably because a lot of Aramcons are going there nowadays)
