Probably I am being pessimist here. But having every thing: BI, Medical, SAMSO clearance etc. Why they insist that we should not resign, while waiting for vista le? Are there any chances that they may change their mind? has same thing similar happened before?

Once a le is available and your visa application is sent to the consulate, my understanding is that the consulate approval step could be still a pass/fail step. Until the Visa is approved nothing is solid

As jjoseph says, there is always a chance that the government might decide not to issue the visa - or even if they do, there is the possibility that it might be delayed, a delay that could last weeks or even (in extreme cases) months.
Hence the advice not to resign until the visa is issued.

In fact, if you look at the thread enled "Visa le " you will see a classic example of why you should not resign from your job too soon. One of the contributors on that thread accepted an offer back in July but still does not have a visa, due to delays in the issuance of a block visa containing the appropriate job le. Imagine if that person had resigned from their old job in July, and was still waiting for their visa - they would have been without a job for around 3 months. Various other "timeline" threads will give varying examples of how long it can take from accepting the offer to getting your visa.

On the forum there an awful lot of threads about timelines. I can't say for sure - I seem to recall some people that failed the medical or BI after accepting the offer but I'm not sure what you mean by "final stage."
You could do a search on "offer" and see what comes up.

Just as an example, I interviewed on April 30, was given a preliminary intent to make an offer May 1st, received and signed the offer August 9th, and received notification requesting our passports October 1st. After that, when we thought everything was in order, my visa le had been issued with the wrong gender. Because of that clerical error, my visa process has been halted until a new block, le, etc. are available. Forty days have elapsed since. I am really glad I didn't resign!!!
