YES! You need your iqama to apply for an exit/re-entry visa.

Americans can apply for a five-year multiple exit/reentry visa for non-iqama holding family members, such as students or spouses who don't live with the employee in KSA. This was discussed elsewhere in the forum.

Just to avoid confusion, the five year visas are not exit/re-entry visas. Exit/re-entry visas are for residents with iqamas only.
The five year visas are multiple-entry visitor visas.
