I've read a few of these with people asking about specific conditions/issues and the answer always seems to be "Depends".
My offer should be arriving this week.
Both my wife and I are very healthy: I could stand to lose maybe 20-30 pounds.
Any idea on what the "knock outs" would be for the medical? I'm assuming serious issues such as cancer, major heart issues etc.

The answer is still depends. It is up to the individual Aramco Doctor who reviews the medical information.
But the conditions you have listed will probably not be an issue.

Yes, a comprehensive medical will be carried out in Saudi for the issuance of iqama. Stay fit.

Anyone with any pre-existing psychiatric conditions will NOT receive medical clearance and will have their offer rescinded. Saudi Arabia does NOT issue work visa/Iqamas for expats with pre-existing psychiatric conditions.
Furthermore, anyone who tests positive for Hep A, B, C, Cholera, Malaria, Syphilis, HIV/AIDS, and TB, will be deemed unfit and NOT receive medical clearance.
Also, varicose veins can be an issue, given the weather conditions and walking that is required.
Basically, all candidates should be reasonable fit, being overweight or obese is fine, however, usually both come with additional health conditions, hence, the medical examination.
Existing conditions such as Cholesterol, Diabetes, etc, that are managed with medical documentation are acceptable.
